2023 Rural CHW Conference CHW/Student Scholarship Form

This form is for students and community health workers (CHWs) who want to apply for a registration scholarship to attend the 2023 Rural CHW Conference on September 29th in Bemidji, MN. Please read this information carefully before beginning the application.

The application is due at 11:59pm on September 15th.  Scholarship recipients will be informed by September 20th. The scholarship is for registration ONLY. The CHW/student is responsible for his/her transportation and lodging for the conference.

We have a limited number of scholarships available. Scholarship funds will be awarded based on the following:

• Availability of funds
• Number of scholarship applicants
• Geographic location (we aim to have diverse attendance from across the state)
• Financial need (please seek funding from your organization before submitting this form. We want to reserve scholarships for CHWs who would otherwise not be able to attend)
• Students must be enrolled in an approved CHW curriculum program.

The application is due at 11:59pm on September 15, 2023 at 11:59 p.m.. Scholarship recipients will be informed on September 20th.
